FINAL FORMS REGISTRATION

All required athletic documents must be completed at FinalForms.com.

• Returning 8th grade Families: Please update and sign existing documents.
• 7th Grade Families: If your athlete was an SCM student in 6th grade, an account already exists. Please do not create a new one to avoid data errors. When searching, select Mishawaka City Schools.
• New to District: Please contact Mrs. Fisher (574-254-3607) or Mrs. Kincses (574-254-3600) for assistance with manual account setup.

Important Note Regarding Physicals: No athlete will be permitted to participate in sports activities without a current physical on file. For your convenience, you may upload your physical directly to FinalForms.com, submit it on the first day of practice or tryouts, or drop it off at the JYMS main office.

PARTICIPATION FEE & REGISTRATION

All JYMS student-athletes participating in a school-sponsored sport are required to pay a participation fee. The fee for next school year has not yet been announced. The current fee is $25 per sport, due the Friday before the first contest.

Sports registration is completed online at: www.mishawaka-in.finalforms.com.

Registration must be completed before the first practice after team selections are finalized. Students may not participate in practices or contests until all forms are completed.

PARENT COMMUNICATION

Once your child becomes a middle school student, all school communication will be sent through ParentSquare.

Please ensure your email address in Skyward is correct so you receive school communications.

Until then, important dates will be posted on the school below.
